- Early 2000s: It gained widespread use on platforms like Something Awful, Fark, and early Reddit
- 2005-2010: NSFW became standard internet vocabulary as workplace internet usage increased and employers began monitoring employee browsing

The phrase reflects a specific historical moment when:
- Office workers increasingly had internet access at their desks
- Employers began implementing content filters and monitoring
- People browsed the internet casually during work hours but needed warnings about risky content
- Workplace harassment and hostile environment lawsuits made viewing inappropriate content at work a serious concern

NSFW has spawned variations:
- NSFL (Not Safe For Life) - extremely disturbing content
- NSFA (Not Safe For Anywhere) - similar to NSFL
- SFW (Safe For Work) - the opposite designation
